Mayor's Celebration of Diversity Award Nomination Form
Instructions for Completing the Nomination Form:
If the nominator is not a member of the organization or business being nominated, he/she must work with the nominee in order to obtain the necessary information to complete the form. Submit each nomination by mail to the Mayor's Celebration of Diversity Awards, 200 E. Washington Street, Rm. 1501, Indianapolis, IN 46204; by fax to 317-327-4482; or by email to lcofer@indygov.org.
Any supplemental information, including letters of support, should be included with the nomination form.
Deadline is 4 pm, Thursday, October 30, 2008. Click one of the links below for the Nomination Form in either .pdf or .doc format. Forms may be mailed, hand-delivered to the address above, or emailed to lcofer@indygov.org, but must be completed in full in order to be considered.
