You may obtain a Voter Registration form at the Marion County Board of Voter Registration, (317)-327-5040, branches of the Bureau of Motor Vehicles, Township Trustee Offices and the Indianapolis/Marion County Public Library branches. You may complete them at home and mail or deliver them to the Board of Voter Registration yourself. If you need assistance filling out this form, the employees of the above agencies will assist you, or call Voter Registration (317-327-5040). This same form may be used for a change of name or address.
CHANGE IN VOTERS REGISTRATION FORM:
As of July 1, 2001, it is now necessary to have the individual registering to provide their Indiana driver's license number. If they do not have an Indiana drivers license then they will need to provide the last four digits of their social security number.
