How to Fill the form out Online

3. Click print when finished.
4. Sign the form.
5. Attach necessary documentation and a check made payable to the "City of Indianapolis" for the necessary filing fee.
(If you are not certain what documentation and fees are necessary to accept your application, please contact IHPC staff at 317-327-4406. Please note that applications submitted without documentation and fees are NOT complete and will not be accepted. It is the responsibility of the applicant to submit a complete application in order to be placed on the IHPC docket for review.)
