In addition to the commodity classes listed with the Vendor Registration Form, there is a detailed list of commodities. Click on the links below to select those specific commodities you are able to provide. Click on the specific commodity class number and you will be able to see all the commodities associated with a particular number.

When completed, the Vendor Registration Form, W9 Form, Commodities for Parts and Goods Form and/or the Commodites for Services Form may be mailed to the Vendor Specialist, City of Indianapolis Purchasing Division, 1522 City County Building, 200 East Washington Street, Indianapolis, IN 46294 or faxed to (317) 327-4493. Please make sure all forms are included with your submission. It is critical that the commodities form is return.
